Implantless osteosynthesis of lateral end of clavicle fractures using autologous palmaris longus tendon graft for coracoclavicular ligament reconstruction

Background: Lateral end clavicle fractures require operative management because of significant displacing forces trapezius, sternocleidomastoid and weight the arm. Coracoclavicular ligaments are usually torn hampering vertical stability. Currently used implants for osteosynthesis associated with implant impingement on acromion, poor purchase lateral fragment cut-out. There is a lack consensus regarding ideal treatment these fractures. Methods: 31 patients fracture who were admitted from May 2019 to August 2022 enrolled in this study underwent coracoclavicular ligament reconstruction using ipsilateral palmaris longus graft looped around coracoid tied over hold it reduced. Patients assessed pain, shoulder range motion functional outcome 6-month follow-up. Results: All showed radiological union good excellent 79.3% mean abduction 157⁰. Mean duration surgery was 104 minutes (SD±12.118) 10.3% stiffness. Conclusions: The shows encouraging prospect purely biological at par other methods fixation while avoiding related complications.
